PRINT AND SIGN -- This form must be submitted, with the signature of both Student and a Parent/Legal Guardian, no later than the first day of class. Students entering 3rd thru 8th Grade and parents/legal guardian must read Acceptable Use Policy (AUP) for Technology (3rd thru 8th Grade), prior to signing this form.

PRINT AND SIGN -- Students, entering Kindergarten thru 2nd Grade, must submit this AUP for Technology by the first day of class each school year, signed by a Parent or Legal Guardian.

PLEASE READ -- Parents/Legal Guardian, please read this Acceptable Use Policy for Technology for 3rd thru 8th Grades thoroughly. After your child(ren) and you have read it, print and sign the "BTS AUP Acknowledgement Form" found on this website.
