If your child will be trying out and/or participating in Sports and/or Dance, the Athletic Physical Evaluation Form must be filled out and signed by a Florida licensed physician and be on file in the School Office before tryouts. This form must be submitted annually.

Also, the Consent and Release from Liability Certificate must be signed by both parents and/or legal guardian(s).
