New Arrival & Dismissal Procedures |

We would like to thank you for making this morning’s arrival smooth and safe. Our students really enjoyed the camaraderie with their classmates and watching the announcements together as a student body. Please remember that upon entering the parking lot, students should unbuckle their seatbelts and be ready to exit their vehicles with lunchboxes and bookbags. Please move up to the 2nd section so that we may unload 4 to 5 vehicles at a time.

It brings us great pleasure to advise you that our plans for the new building/construction are moving forward. We have been advised that the demolition phase of the construction is set to begin the week of January 16, 2012. We have also been advised that the arrival and dismissal procedures must be altered, keeping children and parents free from these areas. As such,

Immunizations & Health Physical |

|
All students attending school in the State of Florida must submit a current Florida Certification of Immunization (Form DH680), completed by a Florida Licensed Physician or Florida Health Department.
All students initally enrolling in a school in the State of Florida must submit the above Form DH 680 and a State of Florida School Entry Health Examination (Form 3040) completed within the past 12 months.
Students entering 7th Grade must submit a current Florida Certification of Immunization (From DH680d), completed by a Florida Licensed Physician or Florida Health Department, indicating that immunizations are complete for 7th Grade. |

REMINDERS: Contact Information Changes: Parents must notify the School Office, in writing, of any changes to Contact Information (e-mail, Phone numbers, address, etc.).
Parents must provide a note from the Doctor/Dentist or Health Facility to the School Office in order for a tardy, early dismissal or absence to be considered excused.
Virtus Certificate: All parents/volunteers must submit their Virtus Certificate of Complete or Re-Certification to the School Office. Please note this requirement is in addition to fingerprint clearance. |
